a 57 year old male with diabetes and hypertension.

This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. 

Here we discuss our individual patient' problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs.

A 57 years old male Came to the OPD complaining of Right knee pain Since 3 years radiating towards right ankle.
The pain appears when he walks and climbs a minimum of one flight of stairs. He was on painkillers for two years which helped him subside the pain momentarily which reappeared shortly.

The patient was apparently asymptomatic 7 years ago then he was diagnosed with hypertension and was on medication.
Six years ago the patient developed paralysis of mouth during his sleep and it sustained for three days and for which he used a 6months
course of Homoeopathy medication.There was no difficulty in his lifestyle or work due to the paralysis.

Afterwards patient was diagnosed with diabetes mellitus and he was suggested with diabetic diet which included one serving of rice and two servings of roti per day with vegetables and low sugar intake .

Patient suffered from fever and loss of appetite (16days) two years ago for which he was treated with antibiotics and paracetamol during which phase he lost weight in a period of one month.
He claims that he has put on 10% of the lost weight after he recovered from the Fever.
patient complains of yellow colored urine since five months.
Patient is not a smoker,alcoholic and tobacco consumer.
Shift towards right when he is walking in a straight line.
Flexion of knee joint minimal since one year.
Family history
COD of his father - Blood cancer leukemia
Mother - Diabetic and HTN since 34 years.

No history of past surgeries
No history of pedal edema.
Sleep : 12am to 7am
Bladder : 5 to 6 times/day; +1/+2 between sleep
Bowel : regular (1/day)

Vitals (7:30 pm; 27/05/22)
BP : 140/80
PR : 76
RS : BAE +
GRBS : 178mm/dl.
Pupils : Reactive.
